Parathyroid hormone (PTH) helps regulation of calcium in the body and maintains high calcium levels in the blood, so that blood calcium remains normal. It is part of a collective system of several chemicals such as calcium, PTH, vitamin D, and, to some extent, phosphorus (phosphate) and magnesium that work together to regulate calcium levels in blood- known as a feedback loop. Conditions and diseases that cause interruption in normal working of this feedback loop can cause inappropriate elevations or decreases in calcium and PTH levels and lead to symptoms of hypercalcemia (high calcium in blood) or hypocalcemia (low calcium levels in blood), respectively. This test measures the amount of PTH in the blood.

If both PTH and calcium levels are normal, then there are chances that the body's calcium regulatory system is working well.
PTH is produced so that it can secrete more calcium in blood if it senses low calcium levels. Low levels of PTH may be due to conditions causing excess calcium to remain in the blood, because of which the body doesn’t find the need to produce enough PTH, or it’s due to an abnormality in PTH production causing hypoparathyroidism.
